The DR In-home Family Support Specialist is responsible for providing DR in-home family support services, crisis intervention including assessment and stabilization of immediate crisis, resource linkages, parenting education, coordinating with multiple service providers to prevent abuse and out–of-home placements; prepare and submit data and reports and attending required meetings and trainings as needed.

  1. DUTIES AND RESPONSIBILITIES:
    • IN-HOME VISITATION:

·       Conduct family assessments; provide specific resources and interventions

·       Provide one-on-one support to families and individuals in home, office or community

·       Provide crisis intervention to assist family in stabilizing situation

·       Prepare and facilitate a variety of lesson plans include coping skills, stress management, and parenting skills in home

·       Assist families in identifying/reaching appropriate community resources

·       Closely communicate clients’ needs, progress to referring SSA social worker
 

  • DOCUMENTATION:

·       Maintain accuracy in case files and notes (including consents, etc.)

·       Maintain files and input data entry on all clients, provide statistics as requested

  • COMMUNITY:

·       Maintain high profile in the community and at the Family Resource Center

·       Maintain/provide current resource referrals within the community

·       Develop/maintain effective working relations with all individuals and agencies impacting program within the community

II. PREREQUISITES

  • Bachelor’s degree in social work or related field from an accredited university required; Master degree preferred
  • Two years experience in working with children and families in crisis situations such as domestic violence, child abuse and neglect.
  • Knowledge of child development and community resources desirable
  • Ability to relate to people of various ethnic and socioeconomic backgrounds
  • Good organizational, follow-through skills and excellent verbal/written communication skills
  • Ability to work in a multi-cultural environment- Valid California “Class C” driver’s license required
  • Bilingual English/Spanish required with proficiency in English required

REPORTS TO : Program Supervisor or Above

III. GENERAL:

  1. Maintains confidentiality in matters of agency operations, personnel, and clients
  2. Maintains positive communications with all agency staff, clients and visitors
  3.         Supports existing agency policies, principles, and mission
  4. Exercises good judgment in performance of duties and responsibilities
  5. Other related duties as assigned
